This Privacy Policy and HIPAA Notice of Privacy Practices (the “Policy”) explains how Med Pro Synergy, LLC and Your Health DPC, PLLC (collectively, “we,” “us,” or “our”) collect, use, protect, and disclose information about our patients, clients, and website visitors. This Policy applies to all individuals who interact with www.medprosynergy.com, receive healthcare or wellness services, or otherwise engage with our organization.
1. Entities Covered
Your Health DPC, PLLC is a physician-owned medical practice with majority ownership (51%) held by a board-certified Medical Doctor and minority ownership (49%) held by a board-certified Physician Assistant. The Medical Doctor retains full and final authority over all clinical decisions and medical care provided through the practice in compliance with Texas Corporate Practice of Medicine (CPOM) laws. Med Pro Synergy, LLC provides management, operational, and administrative services to Your Health DPC, PLLC, and independently offers certain wellness, diagnostic, and preventive services under physician delegation and standing protocols approved by the Medical Director of Your Health DPC, PLLC.
2. Information We Collect
We collect personal, health, and payment information as necessary to provide services and operate our business. This includes:
- Personal identifiers (name, address, phone, email)
- Health and medical information shared through forms, appointments, or communications
- Payment details for self-pay transactions (credit/debit cards, HSA/FSA)
- Technical and analytical data through cookies and website analytics (e.g., Google Analytics).
3. How We Use and Disclose Information
We use and disclose information only as permitted by law and this Policy. Uses include:
- Treatment and coordination of care
- Payment processing and operational management
- Compliance with legal or regulatory requirements
- Communications and scheduling.
We do not sell, rent, or market patient or visitor information to any third parties.
4. HIPAA Privacy Practices
Med Pro Synergy, LLC and Your Health DPC, PLLC both comply with the Health Insurance Portability and Accountability Act (HIPAA), the Health Information Technology for Economic and Clinical Health (HITECH) Act, and applicable Texas privacy laws. Both entities are committed to safeguarding Protected Health Information (PHI) and ensuring confidentiality, integrity, and security in every form of data collection, use, and disclosure. PHI may be used or disclosed for treatment, payment, and healthcare operations, including coordination of services provided by Med Pro Synergy, LLC under physician delegation and protocols approved by the Medical Director of Your Health DPC, PLLC. Examples include laboratory testing, point-of-care diagnostics, Styku body composition analysis, IV and injection therapies, and Thinnr wellness programs. Both organizations implement administrative, technical, and physical safeguards to prevent unauthorized access or disclosure of PHI, and maintain Business Associate Agreements (BAAs) with all vendors who handle health data.
You have the right to:
- Access and obtain copies of your medical records
- Request corrections or restrictions on use or disclosure
- Receive an accounting of disclosures
- Request confidential communications
- File a complaint without fear of retaliation
